• Observe changes, record relevant information promptly and inform the responsible specialists through the agreed channels when something requires attention or further assessment.

• Prepare structured handovers so that observations, risks and agreed actions do not disappear between shifts or remain only in one person's memory.

• Work with colleagues and other involved professional groups, bringing observations from everyday care into case discussions and helping the team maintain a shared understanding.
